Curriculum vitae – You must supply a full work history CV. Your CV must start from the date you left compulsory education and please explain any gaps in your employment. The months and years of your employment must be provided for the past 10 years of your work history, and just years for all other work history. If you have worked ad-hoc assignments via an agency, then please list the name and hospitals and wards where you have attended assignments

References – please provide details for all referees that cover the last 3 years of your employment history, they must be clinically based and must be obtained from your line manager or HR

Qualifications – you must provide all relevant original qualification certificates

Professional registration – You must provide your NMC statement of entry, GMC certificate, IMC certificate or HCPC certificate or card

Right to work in the UK – You are required to evidence your right to work in the UK. Passport or biometric residence permit

DBS certificate – if you are a member of the update service you must supply the DBS certificate you used to join the service. If you are not a member you will be required to complete a DBS application.

2 proofs of address – both documents must show your current address, such as utility bill, bank statement or driving license. They must be dated in the last 3 months

National Insurance Number – NI card, P45, P60m or letter from the jobcentre plus

2 recent photographs – must be colour and passport size

Appraisal – details of your last annual appraisal

Application form – please complete all sections and sign & date the declaration

Medical Questionnaire – please complete all sections and sign & date the declaration

Handbook declaration – please read then sign and return declaration to confirm you have read and understood the handbook
Evidence of professional indemnity

You will need to provide evidence of the following:

  • Hepatitis B titre levels – must be dated within the last 5 years or be accompanied by evidence of a booster
  • Tuberculosis – provide evidence of a BCG vaccination, BCG scar check, heaf or Mantoux test showing evidence of immunity
  • Measles – evidence of 2 vaccinations or a positive serology result
  • Rubella – evidence of 2 vaccinations or a positive serology result
  • Varicella – evidence of 2 vaccinations or a positive serology result. We will also be happy if you self declare that you have had chickenpox

If you will be carrying out exposure prone procedures you will also need to supply blood reports for:

  • HIV
  • Hepatitus C
  • Hepatitus B surface antigen

You must supply original certificates if you have attended the following training:

  • Information Governance
  • Lone Worker
  • Complaints Handling
  • Control & restraint (mental health only)
  • Mental Health Act and Mental Capacity Act
  • Food Hygiene (HCA only)
